Lewistown serves as the county seat.

On September 19, 1789, the county was formed by combining parts of Cumberland and Northumberland counties. It was named after Thomas Mifflin, Pennsylvania’s first governor.

The county covers 415 square miles (1,070 km2), of which 411 square miles (1,060 km2) is land and 3.7 square miles (9.6 km2) is water (0.9%).
