Fredericktown is the county seat and the largest city.

On December 14, 1818, the county was formally established and named after President James Madison.

Mining has long been a major industry in this region, with Madison County boasting the oldest lead mine west of the Mississippi.

The county has a total area of 498 square miles (1,290 km2), of which 494 square miles (1,280 km2) is land and 3.2 square miles (8.3 km2) (0.6%) is water, according to the United States Census Bureau.
