Medicine Lodge is the county seat and the most populous city.
It was named after Thomas Barber, an abolitionist murdered in Douglas County during the Wakarusa War in 1855.
The county has a total area of 1,136 square miles (2,940 km2), of which 1,134 square miles (2,940 km2) is land and 2.1 square miles (5.4 km2) (0.2%) is water, according to the United States Census Bureau.
