Matatiele sits high on the Drakensberg foothills, bordering Lesotho to the north. The terrain here is the highest and coldest in Alfred Nzo District, with several peaks above 3,000 metres along the Lesotho boundary and snow common on the higher ground in winter. It falls under Alfred Nzo District Municipality, adjoining Elundini municipality to the south-west and Greater Kokstad in KwaZulu-Natal to the east.

Matatiele’s provincial history is unusually tangled. The town moved from Cape Province to Natal in 1978, then followed KwaZulu-Natal into being when that province was formed. In 2005 it was moved into the Eastern Cape under a constitutional amendment, over resident objections; the Constitutional Court briefly ruled the move invalid in 2006 for insufficient public consultation, before a corrected process confirmed Matatiele’s place in the Eastern Cape and Alfred Nzo District.
The municipality is one of four making up Alfred Nzo District, alongside Winnie Madikizela-Mandela, Umzimvubu and Ntabankulu, and its border-town economy sets it apart from its poorer, more rural district neighbours.
